NEW SERVICE! – Virtual Diabetes Self – Management Education and Support Program
Looking for support to help in your diabetes management journey to reach that target A1c? Why not try our hybrid virtual class series. You will attend virtual one on one sessions where you can get acquainted with your diabetes educator, while you develop your education plan, create behavior goals and schedule follow up meetings to monitor progress and tweak areas of concern. It takes a village and we are here for you!!
Program Options
OPTION 1 – Hybrid
This option offers TWO one-on-one and TWO group Diabetes Self – Management Education and Support sessions.
Session 1: One-on-one virtual meet and greet.
Meet your diabetes educator, learn about the program, and set your individual health goals.
Session 2: Group virtual class includes:
- What is &Types of Diabetes
- Risk Factors
- Hyper and Hypo Glycemia
- Monitoring
- Sick Day Management
- What is the A1c and Ketoacidosis
- Medications
- Complications Associated with Uncontrolled Diabetes
- Coping and Stress Management
Session 3: Group virtual class includes:
- Physical Activity
- Nutrition
- Support
Session 4: One-on-one virtual follow up and next steps.
Meet with your diabetes educator to discuss your questions about the class content and next steps toward achieving your goals.
OPTION 2 – One-on-One
This option offers FOUR one-on-one Diabetes Self – Management Education and Support sessions. All four sessions follow the pattern above (see option 1) in a one-on-one setting with your diabetes educator.

Need support after class? Struggling to understand the material? Looking for some guidance or motivation? Reach out between sessions to ask for help, your diabetes educator is committed to your success!

Sessions are covered by most insurance carriers*
Physician Referral Required
ALL SESSIONS MUST BE COMPLETE TO BE COVERED BY INSURANCE
*Co-Pay may be required depending on your insurance coverage. You are responsible for 100% of program fees if you do not complete the series.
